What is acne and why does it occur?

Acne is a skin disease suffered by millions of people around the world. Normally, we will see it in teenagers, being more common among young people, but we can also find this disorder in people of all ages.

Acne occurs when an oily substance blocks the hair follicles, that is, sebum or dead skin cells clog these follicles. This disorder can cause several forms of manifesting itself such as whiteheads, blackheads, pimples, depending on whether the pore is still closed or open. If there is the presence of a bacteria, it can cause it to become inflamed and infect the area, generating more severe acne.

Types os acne

Depending on the severity of the acne and whether or not there are inflammatory lesions, we can find the following signs:

  • Without inflammatory lesions there are:
Whiteheads or closed comedones: they are small, very fine, pale-colored accumulations of fat. The pore is still closed and inside it is dirt, oil and dead cells. Their color is due to the fact that they have not come into contact with air, that is, they have not oxidized.

Blackheads or pimples: They are keratin and lipid plugs that obstruct the follicle. This type of acne resembles whiteheads in that it is also a clogging of the skin's pores. However, in this case the pore is open, which causes it to be in contact with the air and oxidize the cells.

When there is an inflammation that affects the area, the different types are presented:

  • Red and small sensitive irregularities: they are papules, that is, lesions on the skin. They have a circumscribed, elevated appearance, well-defined edges, and their content is solid. They stick out above the skin and are usually pink in color and are painful to touch.
  • Pimples or pustules: They are papules but with pus on the tip. They are filled with fluid or pus that may appear on top of an inflamed area of ​​skin. Pustules appear when the walls of the affected pore begin to open and tend to be larger than the whiteheads and blackheads.
  • Large lumps or nodules: They form as larger, more solid lesions within the dermis of the skin. They are painful. They are more difficult to treat. This is also considered part of inflammatory acne. They are deeper within the layers of the skin and rarely point to the surface.
  • Cystic lesions: These are painful, pus-filled lumps under the skin. They are rare and the vast majority benign.

From different studies, to explain the formation of acne it has been divided into four different phases.

    • The first phase begins when there is an increase in the secretion of sebum from our body. This is usually in stages in which a hormonal imbalance appears such as puberty, menstruation or menopause. The hormone called dihydrotestosterone increases and this causes the sebaceous glands to produce more sebum than our skin needs, creating oily skin or with oily areas. The sebaceous glands are connected to the pores of the skin through the follicles. Through these channels, sebum transports dead skin cells to the surface of the epidermis in order to eliminate them.

    • The second phase occurs because the dead cells on a skin or an oily area cannot be shed. Then there is an obstruction of the hair follicle because they accumulate together with the sebum. In this way, our skin begins to generate clogged areas in which sebum and dead cells accumulate.

    • In the third phase, the follicle has grown larger and its obstruction is better appreciated as sebum production continues. If this set of sebum and dead cells is in contact with the air, it will oxidize, giving rise to the black dot. These lesions are not inflammatory.

  • The fourth phase happens if this environment lacks oxygen. This causes the natural balance of the bacterial flora to be disturbed and the propionibacterium acnes bacteria proliferate in excess. This bacteria is linked to acne and the excess oil on the skin acts as a food source for its growth. It is slow growing and is typically anaerobic aerotolerant. This pathogen releases inflammatory substances, giving rise to the appearance of various skin imperfections, which lead to different degrees of acne, since the immune system responds to the growth of this bacterium by forming papules, pustules, nodules and cysts. In this case, they are inflammatory acne lesions.

Causes of acne. Why do we have acne?

      • Hormonal change, as we have already mentioned puberty, pregnancy, menopause.
      • Environmental factors: cities with a lot of pollution, riding motorcycles close to heavy traffic
      • Stress
      • Polycystic ovary syndrome (one of its effects is excessive acne that appears on the face, upper back and even shoulders)
      • Use of some make-up or cosmetic products
      • Some contraceptives or medications

    What should we do to reduce or eliminate acne?

    • We must clean our skin well
    • It is advisable to use products that do not alter the pH of the skin.
    • We must remove the makeup or if we do not have to simply wash with an anti acne soap, neutral soap or micellar water.
    • It is not recommended to use the soap that we use for the hands.
    • Do not wash your face excessively, it is also counterproductive
    • Use products to reduce or eliminate acne

    The most common areas in which acne affects are usually on the face, chest or upper back. Depending on its severity, it can leave scars on the skin.

    This pathology does not affect only on a physical level, it can also cause emotional suffering, damaging self-esteem and levels of self-confidence.
